Job Description & Requirements Specialty : Care Manager Discipline : RN Start Date : 11 / 24 / 2025 Duration : 48 weeks 40 hours per week Shift : 8 hours, days Employment Type : Local Contract Job Title : RN – Case Manager Location : Santa Fe, NM 87505 Duration : 32 Weeks Shift : 8 : 00 AM – 4 : 00 PM | Monday – Friday (5x8s, 40 hrs / week) Responsibilities : Coordinate patient care across departments to ensure continuity and quality of care. Conduct assessments, develop individualized case management plans, and monitor patient progress. Collaborate with interdisciplinary teams, patients, and families for safe discharge planning. Ensure compliance with IHS policies, federal regulations, and patient rights standards. Educate patients / families on health management, resources, and follow-up care.
