Rounding Physician (MD) - Assisted Living Visits

Job Description

Seeking a dedicated MD or DO primary care physician to provide on-site, relationship-driven care in senior living communities across the Greater Twin Cities area.

• Conduct regular rounds in assisted living and memory care settings.
• Perform comprehensive assessments and manage complex health conditions.
• Create personalized care plans collaboratively with patients and families.
• Coordinate with care teams to ensure seamless service.
• Educate residents and loved ones on care options and goals.
• Proactively manage chronic conditions to reduce hospital visits.

Qualifications include active Minnesota medical license, preferred board certification in Internal Medicine, Family Medicine, or Geriatrics, with prior senior living or rounding experience. Strong communication skills and a valid driver’s license are required.
