We are looking for a Home Health Registered Nurse to join our team FULL-TIME / PRN and help our patients remain comfortable, at home .

This is a great opportunity for a Registered Nurse with home health experience who loves to experience the life-changing impact your nursing work has on your patients!

MONDAY THROUGH FRIDAY WITH MINIMAL ON CALL.

You Will :

• Be responsible for overall planning, providing and coordinating quality care to achieve positive patient outcomes in collaboration with the physician.
• Perform patient assessments (e.g. OASIS) in order to implement the best Plan of Care.
• Supervise the care being provided to the patient by Licensed Practical Nurses and paraprofessionals (Home Health Aides) through regular home visits.
• Communicate with family members and other members of the patient's multi-disciplinary team.
