Emergency Dept Behavioral Health Diversion Coordinator NE

Shift : Monday-Friday; 8 : 30 AM - 5 : 00 PM

The ED BH Diversion Specialist reports to the Supervisor, Care Coordination and provides care to clients utilizing the Robert Young Center principles of Trauma Informed Care. This individual will provide comprehensive care coordination services for the population of focus including adults, children, and / or adolescents with Serious Mental Illness (SMI), Co-occurring disorders (COD), or Severe Emotional Disturbances (SED). This position provides intervention, consultation, coordinates discharge planning with community-based services, and follow clients through the discharge process.
