Required Qualifications : Bachelor's Degree from an accredited Occupational Therapy Program. Or Master's Degree from an accredited Occupational Therapy Program. Or Ph.D. from an accredited Occupational Therapy Program. National Provider BLS - American Heart Association within 30 days of hire. Oregon Occupational Therapist License upon hire.
