Our goal is to keep mixed-Medicaid eligible families visiting the doctor together and to provide continuity of care as individuals fall in and out of eligibility.

The Community Health Lead is responsible for overseeing and coordinating the activities of the Community Health Navigators. This position involves leadership, program development, and collaboration with community stakeholders to address health disparities and improve the overall health and well-being of our patients. Additionally, the manager, through the team of navigators will have direct ownership over patient retention and satisfaction.

Supervision & Leadership

• Provide leadership, guidance, and support to a team of Community Health Navigators
• Conduct regular team meetings and trainings to enhance skills and knowledge
• Facilitate cross-cultural communication training for the team
• Foster a positive and collaborative team culture

Education :

Bachelor's degree in public health, community health, social work, or a related field

Experience :

5+ Years in community health, public health, etc.
