Role Responsibilities
  • Deliver comprehensive healthcare to patients spanning all ages, from energetic youths to wise seniors.
  • Address chronic conditions, preventive health, acute illnesses, and mental health with a focus on patient-centered care.
  • Perform thorough physical assessments, analyze diagnostic tests, and craft individualized treatment plans.
  • Work alongside care coordinators, mental health professionals, and other specialists to ensure holistic support.
  • Engage in quality enhancement initiatives that elevate patient outcomes and operational success.
  • Leverage electronic health records (EHR) to uphold precise and current patient documentation.
  • Provide care through both in-person visits and virtual appointments to meet diverse patient preferences.
Education Requirements
  • MD or DO from an accredited institution.
  • Completion of an approved Family Medicine residency program.
Certifications
  • Board Certification in Family Medicine.
  • Eligibility for (or active possession of) a valid Indiana medical license.
  • BLS and ACLS certifications are preferred.
Skills and Experience
  • Robust diagnostic and clinical skills with an emphasis on preventive and comprehensive care.
  • Familiarity with value-based care models and a passion for continuous improvement.
  • Superb communication, collaboration, and interpersonal abilities.
  • Technologically adept with experience using EHR systems and telehealth platforms.
