The End of Life Care Specialist will be responsible for providing on-call patient visits, both scheduled and unscheduled, to address urgent physical and psycho-social issues during after-hours shifts. In coordination with the Call Center Triage and On-Call Manager, this role requires effective communication with the on-call team, including Hospice physicians, to establish a safe plan of care.

This includes :

• Implementing plans of care, educating patients and primary caregivers on changes, medication management, patient safety, and symptom management.
• Attending patient deaths as directed, providing emotional support and guidance to family members.
• Reporting off to the triage and primary hospice team regarding patient-related visits, changes to plans of care, and follow-up needs.

To succeed in this role, candidates must have a strong background in Medical-Surgical Nursing, Community Health, Hospice, or Oncology nursing. A valid New York State RN licensure and NYS drivers' license are preferred. Additionally, candidates should be willing to sit for the national board certification CHPN within one year of hire.
