The Patient Care Coordination Associate plays a crucial role in scheduling new patient evaluations and follow-up appointments, managing communications, and coordinating patient care activities.

This is an office-based role with no direct patient interaction.

Location : 6545 Corporate Centre Blvd Suite 240, Orlando FL 32822

Schedule : Monday-Friday : 8am-5pm.

Key Responsibilities :

  • Conduct telephonic outreach and experience calls to patients.
  • Coordinate patient care following directives from physicians, adhering to company policies.
  • Support the Standard Delegation of Orders (SDO).
  • Schedule new evaluations and follow-up visits for Palliative Care providers.
  • Utilize web-based applications to map driving routes for home visits.
  • Build patient referrals within EMR systems.
  • Maintain and update a master list of Palliative / Bridges patients for reporting purposes.
  • Manage updates and changes in SharePoint for patient information.
  • Handle phone calls professionally and route them appropriately.
  • Manage medical records through filing, scanning, and preparation for scheduling.
  • Ensure timely scanning and filing of all correspondence.
  • Process release requests for medical records and maintain appropriate logs.
  • Demonstrate the ability to work independently.
  • Perform additional related duties as assigned.

This is an office environment with no in-person patient contact.

    Required Qualifications :

  • High School Diploma / GED or higher.
  • 2+ years of customer service experience, with proficiency in data entry and strong communication skills.
  • 1+ years of experience or knowledge of medical office procedures and medical terminology.
  • 1+ years of utilizing internet tools such as Google Maps and MapQuest.
  • Advanced proficiency in Microsoft Office applications, including Excel, Word, and Outlook.
  • Preferred Qualifications :

  • Experience in a physician's office, clinic, or hospital setting.
  • Familiarity with EMR systems and / or TruCare.
  • Previous call center experience.
  • Bilingual proficiency in English and Spanish.
  • The salary range for this position is between $16.00 and $28.85 per hour, depending on full-time employment.
