Job Description : The RN Case Manager is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. Shift :  Day Shift 5 x 8s; Rotating days; 08 : 00 to 16 : 30. Required Qualifications :

Two years acute care RN experience required.

Graduate of an accredited nursing program.

License, Certification, Registration

Registered Nurse License (in the state where care is provided)

BLS (AHA)
