The Racial and Ethnic Approaches to Community Health (REACH) program works to implement population-wide strategies to specifically coordinate and promote vaccine (flu COVID-19 childhood immunization) equity initiatives and events focused on Multnomah Countys African-American / Black immigrant community.
The purpose of this position is to work collaboratively between Integrated Clinical Services including Student Health Services to help develop and lead the implementation of the REACH Vaccine Supplemental Grants (Flu and COVID-19) and chronic disease disparities interventions including communication campaigns policy systems and environmental changes.
This position will oversee program operations develop program policies and procedures track data budget and financial actions and provide technical supervision of program staff and volunteers. This position will work closely with the health center leadership staff REACH Program and its partners to develop and expand the existing vaccine and chronic disease interventions to meet the changing needs of the community.
This position requires in-person and on-site work at county buildings for internal and external meetings community events outreach and engagement activities and other instances where the need of the work is best met in-person and on-site. The position is a hybrid telework position and will include some routine work time at the assigned place of work.
As the Program Specialist your responsibilities will include :
Community and Stakeholder Engagement
Provide ongoing outreach to communities with low vaccination rates particularly Black / African immigrant communities to understand their needs and priorities. Use these insights to shape vaccination strategies identify resource barriers and develop solutions that increase awareness confidence demand and access to adult vaccines.
Develop and implement culturally relevant health promotion programs.
Host REACHing US Health Hubs mini-Health promotion literacy camps and community health fairs.
Employ culturally competent practices with Black / African American and African Immigrant / Refugee communities.
Partner with organizations to promote health education and increase access to resources.
Maintain Grant Contract Compliance
Maintain compliance with all grant and contract requirements.
Prepare and submit reports to the Centers for Disease Control and Prevention (CDC) in partnership with REACH Program staff contracted partners evaluators and other key stakeholders.
Oversee and exercise quality control in the review preparation and approval of new and renewal contract compliance.
Team Operations Project Evaluation and Reporting
Analyze program outcomes prepare reports and conduct interviews focus groups and community surveys.
Collaborate with the team to develop and implement quality improvement strategies and enhance program evaluation and reporting strategies.
Ensure accurate and timely reporting to funders and stakeholders and provide guidance and follow-up for events.
Identify quality improvement opportunities and solutions and work in a culturally competent manner with Black / African American and African Immigrant / Refugee communities.
Technical Assistance and Consultation
Offer technical leadership and oversight to staff and volunteers ensuring efficient program execution.
Train and orient new team members fostering a knowledgeable and cohesive workforce.
Develop and regularly update training manuals to reflect evolving health promotion practices.
Coordinate communication strategies to effectively disseminate accurate vaccine information.

Minimum Qualifications / Transferable Skills
Education / training : equivalent to a bachelors degree and
Experience : one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.
Knowledge Skills and Abilities : This position requires advanced knowledge of and experience working with the African American / African Immigrant and Refugee communities. This would be demonstrated by knowledge of the traditions values and customs of the community being served as well as the ability to identify specific examples of experience (professional and lived) working directly with individuals and families who identify with the African American / African Immigrant and Refugee communities.
Preferred Qualifications / Transferable Skills
Ability to work in a hybrid mobile and pop-up community based vaccine setting.
Ability to speak for 6 hours or more per day.
Advanced knowledge and experience working with African American / Black Immigrant / Refugee communities.
Proven ability to engage with diverse community members leaders and organizations to build trust relationships and facilitate collaboration.
Familiarity with health disparities affecting these communities including social determinants of health and systemic barriers to health equity.
Strong understanding of cultural nuances values and historical context of these communities with the ability to integrate cultural sensitivity into program planning and implementation.
Demonstrated experience working directly with these communities in Multnomah County.

Hybrid Telework : This position is designated as hybrid telework meaning you will be working both remotely (from home) and in person at an onsite location. The designation of Hybrid Telework may be subject to change at a future time. All employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.
