Nurse Practitioner - Home Based Primary Care Program

We are excited to announce an opportunity for a dedicated and compassionate Nurse Practitioner (NP) to join our academic home-based primary care practice, supporting approximately 200 homebound older adults across an 11-county region in the Greater New York Capital Region. This position offers a dynamic work environment where you will collaborate closely with a full-time board-certified geriatrician to deliver comprehensive, longitudinal care to medically complex, homebound patients.

As an NP, you will manage a personal panel of around 40 patients and provide essential cross-coverage for an additional 160 patients in the practice. Your role will encompass home visits, telehealth support, and the management of both chronic and acute medical conditions, with a strong focus on geriatric syndromes and care coordination.

Essential Duties and Responsibilities

Clinical Care :

• Deliver primary care services to a panel of approximately 40 homebound patients through in-person home visits and virtual encounters as appropriate.
• Collaborate closely with the geriatrician to co-manage patient care for the entire practice panel of about 200 patients.
• Cross-cover for the remaining 160 patients, addressing urgent clinical needs, medication adjustments, and patient / caregiver inquiries.
• Perform comprehensive assessments and develop individualized care plans that reflect patients' goals, preferences, and functional status.
• Manage acute and chronic conditions with a focus on geriatric syndromes such as falls, dementia, polypharmacy, and frailty.
• Coordinate with caregivers, visiting nurses, specialists, and other home-based service providers.
• Provide palliative and end-of-life care in alignment with best practices and patient wishes.
