Position Summary : Provides high quality nursing care for assigned patients in a scientific and humanistic focus. The Newly Licensed Registered Nurse (NLRN) utilizes a holistic systems approach to protect and promote client wellness and safety in the physiological, psychological, social cultural, developmental, and spiritual domains. Nursing interventions are interdisciplinary, age specific and are directed toward optimal wellness.

Education, License & Cert : The NLRN must be a graduate of a Diploma, Associate Degree or Bachelor of Science in Nursing (BSN) program and have passed the state board examination by the New York State Board of Nursing before beginning employment. NLRNs who are graduates of a Diploma or Associate Degree program or possess a non‐Nursing Bachelor’s Degree must demonstrate completion of a BSN program within 5 years of hire. The NLRN must successfully complete a medication and IV challenge examination, as outlined by the hospital within the orientation period, as well as maintain CPR certification.

Experience : The NLRN enters practice at a Guthrie Hospital through the Newly Licensed Registered Nurse Residency Program. Clinical specialty tracks may be offered at individual hospitals. Registered nurses with less than one year’s experience must be hired through the program and may not be hired directly into an open position on any specific unit.

Essential Functions : 1. Assessment : The NLRN collects data and information relative to the healthcare consumer’s health or the situation A. Involves patient, family, other healthcare providers in holistic data collection. B. Determines priority of data collections by the patient’s immediate condition or anticipated needs. C. Reviews patient record daily including physician progress notes, laboratory values and nurses’ notes. D. Documents data in a retrievable format. E. Utilizes evidence‐based assessment techniques and resources to identify patterns and variances F. Protects the privacy of collected data based on hospital policies 10. Fulfills professional role expectations and requirements. A. Shares knowledge and skills with peers and colleagues by recognizing and utilizing opportunities to teach other staff members, participating in orientation of new employees and collaborating with faculty in clinical experiences for healthcare students. B. Demonstrates a commitment to lifelong learning by assessing and identifying personal learn needs, developing a plan to maintain skills and competence in clinical practice or role performance, and then completing the plan. Maintains professional records that provide evidence of competency. C. Participates in performance improvement activities to initiate changes in nursing practice and in the healthcare delivery system. D. Participates in efforts to minimize costs unnecessary duplication in patient care. 2. Diagnosis : The NLRN analyzes the assessment data to determine actual or potential diagnoses, problems, and issues. A. Identifies actual and / or potential risks to health and safety, as well as barriers to health and uses data to classify diagnose, problems or issues B. Verifies the diagnosis, problems or issues with patient, family and other healthcare providers and prioritizes them based on goals which were mutually established. C. Documents problems in a manner that facilitates the determination of the expected outcomes and plan. 3. Outcomes Identification : The NLRN identifies expected outcomes for a plan individualized to the healthcare consumer or the situation : A. Develops outcomes involving the patient, family and other healthcare providers when possible and appropriate. Outcomes will reflect a timeframe for attainment. The outcomes will provide for continuity and coordination of care which will be modified based on changes in the status of the patient or evaluation of the situation. B. Documented outcomes will be individualized to the patient (age and culturally appropriate, environmental sensitive), list expected outcomes (goals) and include a timeline for goal achievement. C. Use appropriate resources and evidence‐based practice when faced with complex nursing problems. D. Actual outcomes are evaluated in relation to expected outcomes, safety and quality standards. 4. Planning : The NLRN develops a plan that prescribes strategies to attain expected, measurable outcomes. A. Develops a plan of care and identified priorities with the patient, family and interprofessional team which is individualized, holistic and evidence based. The plan considers economic implications and is compliant with current statues, rules, regulations and standards. B. Advocates for responsible and appropriate use of interventions to minimize suffering. C. Priorities and modifies the plan based on assessment findings and other outcome indicators. D. Documents the plan using standard language / terminology. 5. Implementation : The NLRN implements the identified plan. A. Partners with the patient and family to implement the plan in a manner which is safe, effective, efficient, timely, patient centered and equitable. B. Demonstrated caring behaviors to develop therapeutic relationships. C. Collaborates with the interprofessional team to implement the plan and coordinates the coordination of care. D. Provides care across the lifespan which is congruent, holistic and focuses on the needs of the patient while advocating for identified diverse needs. E. Evidence‐based interventions are utilized to achieve mutual goals and outcomes specific to the problem or need. F. Critical thinking and technology solutions are integrated into the implementation of the nursing process to enhance outcomes. G. Delegates appropriately while maintaining accountability for care. H. Document implementation and any modifications, including changes or omissions, for the identified plan. 6. Coordinator of Care : The NLRN coordinates care delivery. A. Organizes the plan. B. Collaborates with the patient and manages their mutually agreed upon outcomes C. Engages the patient in self‐care to achieve preferred quality of life D. Communicates with the patient, interprofessional team and community‐based resources to ensure safe transitions in care E. Advocates for dignified and holistic care by the team F. Documents coordination of care 7. Health Teaching and Health Promotion A. Provides opportunity to the patient to identify health teaching and health promotion needs. B. Utilizes health teaching and health promotion methods to meet the varied needs of patients and evaluates their effectiveness. C. Uses technology to communicate health promotion and disease prevention to patients and provides them with information about potential adverse effects of their plan of care. D. Provides anticipatory guidance to patients to promote health and reduce the risk of negative health outcomes. 8. Evaluation : The NLRN evaluates progress toward the attainment of goals and outcomes. A. Conducts ongoing evaluations which are holistic, systematic and criterion based, in collaboration with the patient and interprofessional team. B. Works with the patient and interprofessional team to determine the patient‐centeredness, effectiveness, efficiency, safety, timeliness and equitability of the plan and attainment of outcomes. C. Revises the diagnoses, outcomes, plan, and implementation strategies based on ongoing assessment data. D. Communicates the evaluation and conclusion of the plan to the patient and other key stakeholders. E. Documents the results of the outcomes. 9. Performs nursing care in compliance with nursing standards of practice, standards of professional performance, the American Nurses’ Association Code of Ethics for Nurses and the organization’s policies and procedures. A. Supports the philosophy of Corning Hospital Department of Nursing. B. Reports and documents accurately change in patients’ condition and response to nursing care. C. Demonstrates competence in both identification of an emergency situation and in initiating appropriate plan of action safely and effectively using emergency equipment. Delegates responsibilities to other staff members and ancillary personnel during an emergency. D. Demonstrates procedures and interprets policy to other members of the healthcare team.

Other Duties : 1. Assumes leadership responsibility when appropriate. 2. Participates in the planning and presentation of educational programs. (Clinical Ladders Nurses) 3. Payrange is from $37.31-$53.33
