We are looking for a dedicated Social Worker (LMSW) to join our client’s multidisciplinary team. This role is essential for supporting patients’ psychosocial needs, conducting discharge planning, and coordinating care within a fast-paced hospital environment.
Key Responsibilities
- Conduct biopsychosocial assessments and plan / implement appropriate patient interventions.
- Provide ongoing supportive counseling and psychosocial interventions to patients and their families / significant others.
- Collaborate and coordinate with the patient's family, community resources, and medical providers to ensure effective treatment and aftercare planning.
- Advocate for patients with social and community agencies to secure necessary services.
- Incorporate universal screening for abuse victims (including child abuse, IPV, elder abuse, sexual assault, and human trafficking) and follow all mandated reporting and hospital intervention policies.
- Complete all required documentation accurately and in a timely fashion.
- Comply with all department and hospital policies, understanding the responsibilities of an essential worker during extreme circumstances.
Qualifications & Requirements
Required Education : Master’s degree in Social Work (MSW).Required Certification : Valid Licensed Master of Social Work (LMSW) license issued by NYSED is mandatory.Experience :Minimum of 1 year of social work experience.Hospital experience is strongly preferred.Experience in Adult Comprehensive Psychiatric Emergency Program (CPEP) is a plus.Shift Time : Working hours are generally between 8 : 00 AM and 10 : 00 PM (4 nine-hour shifts).“We are an Equal Opportunity Employer.
