With over 50 years of community presence, we provide high-quality, patient-centered care. Our MSO organization includes 120+ employees, 16 physicians, and a strong leadership team.

Role Summary :

Position is for a licensed Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to lead our cardiology team in a transformative initiative aimed at improving outcomes for heart failure patients. This role is central to reducing hospital readmissions, enhancing quality of life, and supporting physician workflows through a multidisciplinary, patient-centered care model.

Physician Workflows : Integrate and supplement primary care and heart failure cardiology clinical visits in the management of heart failure patients.

Key Responsibilities :

  • Conduct ambulatory visits for HF patients, serving as the primary cardiology-based NP / PA
  • Provide early post-discharge follow-up (within 3–5 days) to ensure continuity of care
  • Collaborate with cardiologists, nurses, pharmacists, and care coordinators in a multidisciplinary team.
  • Utilize remote monitoring tools and telehealth platforms for high-risk patient surveillance.
  • Educate patients and families on medication adherence, dietary guidelines, and symptom recognition.
  • Coordinate referrals to ancillary services including cardiac rehab, dietary services, discharge planning, and social work
  • Participate in patient education programs and workshops to promote self-care and family engagement.
  • Support palliative care efforts including symptom management and advance care planning
  • Document all clinical interactions and care plans in the EMR with accuracy and timeliness
  • Monitor and report on quality metrics related to readmission rates and patient outcomes

Ancillary Services Integration :

  • Cardiac Rehabilitation : Facilitate referrals and monitor progress
  • Nutrition Counseling : Collaborate on personalized dietary plans
  • Pharmacy Services : Support medication reconciliation and patient education
  • Social Work Services : Connect patients with community and financial resources
  • Home Health Care : Coordinate in-home nursing and therapy as needed
  • Telehealth Services : Engage patients through virtual consultations and remote monitoring
  • Patient Education Programs : Lead or support workshops and resource distribution
  • Palliative Care : Partner with specialists to optimize quality of life
  • Quality Measure Alignment :

    This role directly supports our IPA's strategic goals by targeting 7-day readmissions-a critical quality metric. Early outpatient follow-up and coordinated care are proven strategies to reduce readmissions and improve long-term outcomes.

    Qualifications :

  • Licensed Nurse Practitioner or Physician Assistant in California with national certification
  • Minimum of 2–3 years of clinical experience, preferably in cardiology or critical care
  • Proficiency in EMR systems and remote monitoring platforms; Epic preferred
  • Strong communication, organizational, and patient engagement skills
  • BLS / ACLS certification (or willingness to obtain)
