Job Summary : Care Must is seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will work with patients to improve their ability to perform daily activities and enhance their quality of life. This role involves developing individualized care plans, providing therapy in both outpatient settings and skilled nursing facilities, and collaborating with other healthcare professionals to ensure comprehensive patient care.Responsibilities :
- Assess patients' needs and develop personalized care plans to address their specific challenges.
- Provide therapeutic interventions to help patients regain or improve their functional abilities.
- Assist patients in learning new skills or adapting existing skills to enhance independence in daily living.
- Collaborate with multidisciplinary teams for effective case management and discharge planning.
- Maintain accurate documentation of patient progress and treatment outcomes.
- Educate patients and their families about treatment plans, goals, and home exercises.
- Utilize medical terminology effectively when communicating with other healthcare professionals.
- Participate in early intervention programs for children with developmental disabilities.
- Ensure compliance with all regulatory requirements and best practices in patient care.
- Ability to assist individuals with disabilities in achieving their therapeutic goals.
- Excellent communication and interpersonal skills to work effectively with patients, families, and colleagues.Qualifications :
- Graduate of an Occupational Therapy curriculum.
- Licensed as an Occupational Therapist.
- Minimum of two years of experience as an occupational therapist.
- Preferably at least 1 year of Home Health experience (Training provided for OT without home health experience).
- Must have a valid professional license to practice OT in the State of California.
- Valid CPR License.
- Valid Driver's License and your reliable transportation.
