Role Responsibilities

• Deliver comprehensive, ongoing care to individuals and families across all age groups-from children to seniors.

Education Requirements

• MD or DO from an accredited institution.
• Completion of an accredited residency in Family Medicine.

Certifications

• Board Certification in Family Medicine.
• Eligibility for (or possession of) an active medical license in Indiana.
• BLS and ACLS certifications preferred.
