Job Description

Job Title : RN Case Manager

Location : Grand Blanc, MI

Contract Length : 13 Weeks

Weekly Gross : $2100 - $2200 / weekly

Required Qualifications :

• Minimum 5 years of acute inpatient case management experience , including acute discharge planning .
• Bachelor of Science in Nursing (BSN) – Required
• BLS Certification – Must be current and issued through AHA .
• Active and unrestricted RN license in the State of Michigan .
• Experience with CarePort – Required
• Able and willing to be physically present on patient floors and engage directly with patients.
• Ability to acclimate quickly and work independently with minimal supervision.
