Description :
The Medical Oncology Nurse Navigator is a professional registered nurse with oncology-specific clinical knowledge.
They offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers.
Utilizing the nursing process, the Medical Oncology Nurse Navigator provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.
Working within the Hematology / Oncology Outpatient Practice setting, the Oncology Nurse Navigator provides patient-centered care to patients of all cancer types and benign blood disorders.
The Oncology Nurse Navigator works as part of a dynamic multidisciplinary team of professionals including MDs, NPs, RNs, Medical Assistants, Pharmacists, and Social Workers.
The Oncology Nurse Navigator functions as an integral member of the team helping ensure patients receive quality and compassionate care.
Skills such as clinical decision-making, critical thinking, effective communication, and nursing assessment and symptom management specific to oncology are necessary for this role.
They are responsible for coordinating care for patients for a defined population, including the evaluation, assessment, scheduling, consultation, and treatment of patients to implement a seamless progression of care to meet the patient's needs.
The Navigator provides a welcoming, helpful approach to new patients while promoting a positive and professional image.
Patient and family education regarding chemotherapy / immunotherapy is provided.
Care coordination for patients preparing to start treatment includes pre-treatment staging, labs, other testing, and venous access placement.
Collaboration with colleagues is essential in the care of medical oncology patients.
Support for Hematology / Oncology physicians and nurse practitioners includes telephone triage, medication prescription renewals, and outside lab ordering.
Collaboration with physicians and other health care providers is essential to coordinate care for patients from referral / consultation to discharge and follow-up.
