The Childrens and Womens Health Navigation Coordinator (HNC) is a registered professional nurse who uses an evidence-based, multidisciplinary approach to support consumer navigation through the AdventHealth continuum of care. The HNC assists providers and multidisciplinary teams in navigating patient care pathways. They also assist leadership with program development by acting as liaison with stakeholder teams across the system and providing resources / data. The HNC supports data collection, clinical collaboration, and marketing / community outreach efforts. They may also participate in process improvement activities to achieve shared campus goals. The HNC actively participates in outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.
