Position Overview :

Contract; Contract-to-Hire, Full Time 40 hours / week

Haven Healthcare Staffing is seeking a Physical Therapy Assistant to support our client in an Outpatient Clinic setting. This is a Contract role with the potential to convert to Direct Hire for the right fit. As part of the Interdisciplinary Team, the Physical Therapy Assistant will provide high-quality, personalized physical therapy services to patients in their homes, improving their quality of life and helping them achieve their physical recovery goals. You will work closely with an interdisciplinary team, including physicians, nurses, and caregivers, to deliver exceptional care tailored to each patient's needs.

Key Responsibilities :

• Patient Evaluation :  Assess and evaluate patients' physical abilities, strengths, and limitations through in-depth clinical assessments.
• Develop Treatment Plans :  Create individualized treatment plans based on patient evaluations, medical history, and physician orders.
• Implement Therapy Interventions :  Administer physical therapy interventions such as exercises, manual therapy, and modalities to improve mobility, strength, balance, and overall function.
• Patient Education :  Educate patients and their families on proper body mechanics, exercises, and strategies to improve functionality and reduce injury risk.
• Progress Monitoring :  Track and document patients' progress, modifying treatment plans as necessary to meet evolving needs.
• Collaboration :  Work closely with the patient’s care team, including physicians, nurses, and other health professionals, to ensure comprehensive care.
• Compliance :  Ensure that all treatments and interventions comply with legal, ethical, and regulatory requirements.
• Documentation :  Maintain accurate, detailed, and timely documentation of patient evaluations, treatments, and progress, following company and regulatory guidelines.
• Discharge Planning :  Prepare patients for discharge by assessing functional progress and recommending appropriate follow-up care.
