Position Overview :
Contract; Contract-to-Hire, Full Time 40 hours / week
Haven Healthcare Staffing is seeking a Physical Therapy Assistant to support our client in an Outpatient Clinic setting. This is a Contract role with the potential to convert to Direct Hire for the right fit. As part of the Interdisciplinary Team, the Physical Therapy Assistant will provide high-quality, personalized physical therapy services to patients in their homes, improving their quality of life and helping them achieve their physical recovery goals. You will work closely with an interdisciplinary team, including physicians, nurses, and caregivers, to deliver exceptional care tailored to each patient's needs.
Key Responsibilities :
- Patient Evaluation : Assess and evaluate patients' physical abilities, strengths, and limitations through in-depth clinical assessments.
- Develop Treatment Plans : Create individualized treatment plans based on patient evaluations, medical history, and physician orders.
- Implement Therapy Interventions : Administer physical therapy interventions such as exercises, manual therapy, and modalities to improve mobility, strength, balance, and overall function.
- Patient Education : Educate patients and their families on proper body mechanics, exercises, and strategies to improve functionality and reduce injury risk.
- Progress Monitoring : Track and document patients' progress, modifying treatment plans as necessary to meet evolving needs.
- Collaboration : Work closely with the patient’s care team, including physicians, nurses, and other health professionals, to ensure comprehensive care.
- Compliance : Ensure that all treatments and interventions comply with legal, ethical, and regulatory requirements.
- Documentation : Maintain accurate, detailed, and timely documentation of patient evaluations, treatments, and progress, following company and regulatory guidelines.
- Discharge Planning : Prepare patients for discharge by assessing functional progress and recommending appropriate follow-up care.
Qualifications :
Active and unrestricted Physical Therapist license in Washington State.Experience : 1-2 years of experience as a physical therapist, with 1 year of experience in home health requiredSkills & Competencies :
OASIS CompetencyStrong knowledge of physical therapy techniques, interventions, and evidence-based practices.Excellent communication and interpersonal skills, with the ability to educate patients and families.Ability to work independently and manage a caseload.Strong organizational skills and attention to detail.Compassionate and patient-centered approach to care.Certifications :
CPR / BLS certification.Experience or certifications in Geriatric and / or care of vulnerable populationsPhysical Requirements :
Ability to lift up to 50 pounds and assist with patient transfers.Ability to bend, kneel, and perform physical tasks throughout the day.Ability to travel to patient homes within the designated service area.About Haven Healthcare Staffing :
