Job Description

Specific duties include the following but are not limited to :

• Process, monitor and review all reportable and non-reportable incidents in conformance with Part 624 of 14NYCRR.
• Conduct investigations as assigned by NYS Justice Center and Incident Management Coordinator.
• On-call 24 hours / 7 days week for reporting and investigation of urgent agency incidents.
• Assist in the staff training process regarding incident identification and reporting.
• Interface with Agency personnel and community / service providers.
• Attend monthly Incident Review Committee meetings. Record meeting minutes ensuring adequate description and accuracy. Generate meeting minutes / reports in a timely manner.
• Coordinate with Quality Assurance staff to ensure on-going compliance with federal and state incident management requirements.
• Attend OPWDD facility audits when requested.
• Develop compliance protocol and administrative guidelines relative to in the incident management process.
• Update the incident management data base, as necessary.
• Assist Quality Assurance staff in implementing the internal QA process, as required.
• Perform additional duties as required.
