Case Manager

I’m partnering with a great pediatric home health organization that’s expanding their IHSS (In-Home Support Services) program in Colorado and looking for an experienced  Case Manager to join their team! This hybrid role offers the chance to make a real difference for children and families providing hands-on supervision, care coordination, and caregiver support to help kids safely thrive at home.

Position Highlights :

• Hybrid schedule
• Oversee pediatric IHSS cases and provide in-home supervision
• Conduct skills validation and competency checks for attendants
• Review and update care plans and documentation
• Investigate and resolve client or caregiver concerns Collaborate with families, authorized reps, and care staff
