Location : Tarrytown,NY / Mobile

Hours per Week : 35

FLSA Status : Full Time, Exempt

Salary Range : $80,000 - $95,000 annually

Summary Description : The RN / LPN will work as part of a multidisciplinary team in the Critical Time Intervention (CTI) program serving adults with co-occurring behavioral health and physical health needs who frequent ED / inpatient settings. The RN / LPN is responsible for assessing basic health and medical needs within their scope of practice, coordinating health care with community medical providers (e.g., primary care physicians, specialists, etc.), rehabilitation, education, and support services.

Key Responsibilities :

• Perform nursing assessments and document client medical history and current health status.
• Monitor and record vital signs and other relevant health indicators.
• Collaborate with the healthcare team to develop and implement individualized care plans.
• Provide direct client care, including wound care, injections, and assistance with daily living activities.
• Educate clients and their families on healthcare plans, medications, and self-care techniques.
• Maintain accurate and up-to-date medical records and documentation.
• Collaborate with other healthcare professionals to ensure coordinated and holistic care.
• Respond to medical emergencies and provide immediate first aid and intervention.

Mandated Reporter :

This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter.
