About The Position
Start Date : 11 / 27 or as soon as credentialed Thursday Nov 27, 2025 – Sunday Nov 30, 2025, Friday Dec 19, 2025- Thursday Jan 1, 2026.
Contract Length : 3 months, ongoing
Shift Details : Round on patients M-F. Admit patients to rehab unit. Lead Team Conference Meetings. All inpatient rehab documentation requirements (H&P, 4-day poc, team conference, progress notes – change in status 3x / week, discharge summary).
Schedule : Thursday Nov 27, 2025 – Sunday Nov 30, 2025, Friday Dec 19, 2025- Thursday Jan 1, 2026.
Work Details :
- Support Staff : Nursing. Intake Team, PT, OT, SLP. Specialty Consults
- Amount of Coverage Needed per Month : 7 days as week. 7am – 7pm. Will round on patients daily from 8a-3 / 4p.
- Patients per day : varies on census. Max Up to 18 but average 10-12
- Patient Population / Age : Adult
- Department Description : 18 bed acute rehab unit housed on 2nd floor of acute hospital.
