Admissions Coordinator Transformation House

Were looking for a detail-oriented and personable Intake Coordinator to join our team and help ensure every new client experiences a smooth, supportive start to their treatment journey.

Position Summary

The Intake Coordinator is responsible for coordinating new client admissions and building positive relationships with prospective clients, referral sources, and community partners. This role is key to ensuring an efficient, accurate, and caring intake process that reflects the mission and values of Transformation House.

Key Responsibilities

  • Provide general information about companys programs and admissions process to direct prospects and third party referring agencies.
  • Establish and maintain business relations with referring entities
  • Schedule and perform intake interview
  • Collect prospects documentation necessary for the intake screening
  • Submit to the review team prospect clients file for intake screening (approval for admission).
  • Schedule new client date of admission
  • Submit notification of admission to funder within 24 hours of new client date of illness
  • Monitor and manage wait list
  • Perform necessary data entry into EMR (Procentive)
  • Determine client financial responsibility and monitor funders change (each first of the month)
  • Monitor daily census log
  • Maintain up to date and monitor the intake progression spread sheet
  • Attend staff in services and meetings
  • Other duties as assigned.

Qualifications

  • Minimum 1 year of experience in a substance use disorder treatment facility.
  • 23 years of related experience preferred, or equivalent education and experience.
  • Knowledge of relevant Minnesota statutes :
  • Chapter 245G (Substance Use Disorder Programs)

  • Must successfully complete a DHS Background Study.
  • Strong computer skills and proficiency with standard office equipment (copier, scanner, printer).
  • Excellent organizational, communication, and interpersonal skills.
