Position Overview :
We are seeking an experienced Home Health Physical Therapist to join our Maryland team. The ideal candidate will be dedicated to improving the quality of life for our patients by providing expert physical therapy services tailored to individual needs. This role offers the opportunity to work independently while collaborating with interdisciplinary teams to ensure the highest standard of care.
Responsibilities :
Conduct comprehensive assessments of patients' physical and functional abilities.Develop personalized treatment plans in accordance with physician recommendations and patient goals.Provide skilled physical therapy interventions to improve mobility, strength, balance, and overall function.Educate patients and caregivers on therapeutic exercises, safety precautions, and injury prevention strategies.Monitor patients’ progress and adjust treatment plans to optimize outcomes.Document all clinical findings, interventions, and patient responses accurately and promptly.Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.Maintain compliance with HIPAA, Medicare, and all state regulatory standards.Participate in ongoing professional development to enhance clinical skills and stay current with best practices.Qualifications :
Graduate of an accredited physical therapy program with an active Maryland license.Minimum of 3 years’ clinical experience, preferably in home health or skilled nursing facilities.Strong clinical reasoning and assessment skills, with the ability to adapt plans to patient needs.Experience with diverse patient populations, including adults and older adults.Ability to recognize medical emergencies and respond appropriately.Excellent communication and interpersonal skills.Proficiency with electronic medical records (EMR); Kinnser, WellSky, or Homecare Homebase preferred.OASIS experience preferred.Ability to supervise Physical Therapist Assistants (PTAs).Reliable transportation and a valid driver’s license.Strong time management and organizational skills.Join Our Team :
