Base Salary : $567,000

Sign on Bonus-$75k

Performance Incentive : Quality-5% of base compensation paid per year for achieving defined performance metrics

Productivity Incentive : wRVU threshold 4124 with conversion faction of $137.66 this year

Call Greater than 1 : 3 : Paid at $1000 per 24 hours of unrestricted call

• Relocation Services : Up to $15,000 in approved relocation expenses (grossed up to cover tax liability);

Allowed Time Off 40 days (320 hours) each fiscal year. This allowed time off covers all time off including vacation, holidays, personal days, and continuing education.
