Your Role and Responsibilities :

• Deliver exceptional home-based care to patients recovering from illnesses, injuries, or chronic diseases.
• Collaborate with a team of physicians while overseeing LPNs, CNAs, and aides in providing patient care.
• Conduct thorough assessments, document progress, and communicate changes to physicians.
• Educate patients and their families on care plans, medications, and home safety.
• Ensure that care goals are met and coordinate discharge from services.
