What Youll Do :

As an RN II in Advanced Primary Care, youll work closely with a multidisciplinary teamincluding physicians and advanced practitionersto provide ongoing support to patients with chronic conditions. Your responsibilities will include :

• Building strong patient relationships and monitoring vital sign trends.
• Conducting virtual visits between regular office appointments.
• Supporting chronic disease management, discharge follow-up, and acute care needs.
• Advancing guideline-directed medical therapy (GDMT) and facilitating advance care planning for Medicare and MA patients.
• Collaborating with primary care clinicians in ambulatory settings to coordinate care.
• Performing virtual assessments, patient education, triage, and escalation of care needs.
• Assessing medically complex patients and acute exacerbations of chronic conditions.
• Coordinating care with a multidisciplinary team including specialists, social workers, physical therapists, and palliative care providers.
• Documenting all care provided and maintaining patient confidentiality.
• Practicing within the RN scope under the supervision of an advanced practitioner.
