Community Intervention Specialist

Department of Health & Human Services

Hourly Range : $28.22 - $32.85

Job Summary : Milwaukee County Housing Services (MCHS) is in search of a Supportive Housing case manager. This position is responsible for working with formerly homeless individuals and families as they transition into permanent housing. Individuals taking part in this program are in one of a variety of housing programs within MCHS and living within Milwaukee County. The Supportive Housing case manager will build rapport, model, advocate for and assist individuals in building skills to live independently in the community after a period of instability.

Job Duties :

• Manage caseloads of clients with severe and persistent mental illness and substance use disorders. Act as a liaison between participants, and landlords.
• Advocate and intervene on behalf of clients to maintain housing stability.
• Increase stabilization through support in the form of home visiting and linkage to resources including but not limited to FoodShare, meal sites or food pantries, mental health and / or medical providers, transportation, employment, access to social security, and other long term care programs.
• Work directly with housing providers and participants to ensure timely completion of housing program renewals, intake forms, etc.
• Provide housing navigation services assisting clients with locating apartments as they exit homelessness or are moving to a new unit.
• Assist with transportation, coordination of moves, as well as management of medical and mental health appointments.
• Utilize Housing First principles in their approach to serving unhoused individuals.
• Collaborate with MCHS and great DHHS staff / co-workers to comprehensively serve program participants.
• Attend applicable trainings and meetings.
• Perform other duties as assigned.

Minimum Qualifications :

Knowledge, Skills, and Abilities : Knowledge of housing programs and HUD regulations as they relate to homelessness and income guidelines; knowledge of available housing resources; knowledge of the permanent supportive housing model; ability to conduct clinical assessments to provide for proper discharge planning; ability to work with individuals with a wide range of needs.
