Minnesota Licensed Clinical Pharmacist

A company is looking for a Remote Clinical Pharmacist.

Key Responsibilities

Deliver disease and medication management services via phone or telehealth

Conduct comprehensive medication reviews to identify therapy gaps and medication-related issues

Collaborate closely with internal care teams and external partners to coordinate patient care

Qualifications

PharmD from an ACPE-accredited College of Pharmacy

Active Minnesota pharmacist license or eligibility for licensure

Board Certification (BCGP, BCPS, BCACP) or willingness to obtain within 12 months

Demonstrated independence and sound clinical judgment

Current knowledge of all phases of pharmacy practice
