Virginia Licensed Field RN Case Manager

A company is looking for a Field RN Case Manager.

Key Responsibilities

Manage patient care and coordinate services in a remote setting

Conduct assessments and develop care plans for patients

Monitor patient progress and adjust care plans as necessary

Required Qualifications

Minimum of an associate degree in nursing

Active and unrestricted RN license in Virginia or a compact state

At least 2 years of clinical experience

Experience in a fast-paced healthcare environment
