LPN III Position Details:

• Facility: BayCare HomeCare

• Location: Lakeland

• Status: Full Time

• Shift Hours: 8:00 AM - 5:00 PM

• Weekend Work: Occasional

• On Call: Yes

• Home visits in assigned service area: Polk County

The Home Health LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:

• Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse

• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse

• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status

• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Required Certifications and Licensures:

• BLS (Basic Life Support)

• LPN (Licensed Practical Nurse) License

• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

Education:

• Required Associate's or Diploma Nursing

Experience:

• Required 1 year Licensed Practical Nurse

• Preferred 1 year Home Care
