Responsibilities
Manage patient care plans and coordinate services for optimal outcomes.
Conduct assessments to identify patient needs and develop care strategies.
Achieve performance targets and contribute to team success.
Collaborate with healthcare teams to ensure comprehensive patient support.
Participate in professional development and training opportunities.
Engage in continuous learning to enhance clinical skills and knowledge.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Document patient interactions and maintain accurate records.
Utilize data to track performance and improve service delivery.
Participate in quality improvement initiatives to enhance patient care.
Provide support and guidance to junior staff and new team members.
Experience / Qualifications
Registered Nurse (RN) license in California.
A results-driven mindset with a focus on achieving performance targets.
Experience in case management and patient care coordination.
Commitment to continuous learning and professional development.
Strong assessment skills to identify patient needs effectively.
Ability to collaborate with multidisciplinary healthcare teams.
Ability to monitor patient progress and adjust care plans proactively.
Strong communication skills for patient and family education.
Proficient in documenting patient interactions and maintaining records.
Experience in quality improvement initiatives in healthcare.
