Position Summary

The Client Care Coordinator plays a vital role in delivering exceptional client care and support to our clients and their families. This position requires strong communication, organization, and relationship-building skills, as well as the ability to collaborate effectively with internal team members to ensure seamless care. This is a full-time position, Monday through Friday, from 8 : 00 AM to 5 : 00 PM, with a one-hour lunch break each day.

Key Responsibilities

• Coordinate and schedule client appointments and in-home visits
• Maintain accurate, timely documentation of client information and care plans
• Respond to client and family inquiries professionally and promptly
• Collaborate with internal care teams to support high-quality, coordinated client care
• Assist in developing and implementing effective client care strategies
