Description :
This role involves providing home health physical therapy services.
Responsibility includes visiting patient homes and documenting care provided.
Periodic office meetings are required.
Average number of patients per day is 5-6.
The physical therapist will be assigned between 5-10 patients per day.
Each type of visit and activity holds a different point value toward productivity expectations.
The therapist must be willing to travel up to 75 miles from the home clinic.
Mileage will be reimbursed at the IRS standard rate.
The commute to and from the first and last patient is not billable.
Weekend requirements vary based on location and profession.
The physical therapist will handle start-of-care follow-ups and ongoing visits with patients.
It is essential to complete documentation within 24 hours of each appointment with the patient.
All documentation must be accurate and timely to ensure billing and follow-up patient care.
There are no guaranteed hours, and the clinician must achieve a minimum of 30 productivity points per week.
Declining patients may result in a cancelation of the contract.
The clinician will not be called for on-call duties.
Experience in home health is preferred.
Only candidates with recent home health experience will be considered.
The clinician should be comfortable with the potential for longer drives between patients.
