Revital Rehab Services is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our partner facility, Cedar Haven Healthcare Center, a skilled nursing community in Lebanon, PA. In this role, you’ll support residents in regaining function, increasing independence, and improving their quality of life—all within a collaborative and supportive environment.

Your Impact

Provide direct occupational therapy services under the supervision of a licensed Occupational Therapist (OT)

Assist with screenings and assessments to support the development of personalized care plans

Deliver compassionate, goal-oriented treatments tailored to residents’ individual needs

Maintain accurate and timely documentation and communicate progress with the OT and interdisciplinary care team

What You’ll Need

Associate’s degree from an accredited Occupational Therapy Assistant program

Active Pennsylvania COTA license (or eligibility)

Strong communication skills and a resident-focused approach

Commitment to ongoing learning and professional growth
