Job description

Company Description

PrePost+ is a group Pre-Operative Clearance and Post-Surgical Primary Care Centers which directly addresses the growing primary care physician (PCP) shortage while aligning with the CMS Transforming Episode Accountability Model (TEAM). By offering dedicated post-surgical follow-up care, integrated rehabilitation, and expedited pre-operative clearances, the center fills critical gaps caused by limited PCP availability, reducing surgical delays and rehospitalizations. It offloads burden from traditional PCPs by employing nurse practitioners and physician assistants, and expands access through telemedicine. Strategic partnerships with hospitals, surgical centers, and insurers ensure patient continuity and access. Financially, the model is sustainable through diversified revenue from clearances, post-op care, and rehab services, making it a scalable, TEAM-aligned solution to improve outcomes and control costs.
