Description :
Provide assessment, diagnosis, and treatment of speech, language, cognitive-communication, and swallowing disorders in a geriatric, medically complex population in a subacute / long-term setting.
Collaborate with an interdisciplinary team and support quality patient outcomes and regulatory compliance.
Conduct comprehensive evaluations within 24 hours of physician referral to assess speech, language, cognition, and swallowing.
Develop a functional therapy diagnosis and prognosis.
Design and implement individualized treatment plans, use evidence-based interventions, adjust treatment based on progress, and establish clear, measurable goals.
Provide direct therapy addressing communication and swallowing disorders in residents, including compensatory strategies, cognitive-communication interventions, and group therapy when appropriate.
Document accurately and timely : evaluation reports, daily treatment notes, weekly progress updates, 14-day physician progress reports, discharge summaries, billing documentation, and any plan of care logs.
Educate residents, families, and nursing / CNA staff on communication strategies, safe swallowing techniques, and home-exercise programs.
Coordinate care with the interdisciplinary team including nurses, physicians, dietary, and social work to optimize patient outcomes and support discharge planning.
Participate in quality improvement and facility initiatives; advocate for patient-centered care, ethical practice, and compliance with documentation standards.
If supervising therapy aides, ensure compliance with regulations : supervisor must have 2 years experience, provide direct supervision for 20% of duties for the first 90 days, and complete continuing education in supervision.
Experience in skilled nursing facility or geriatric / rehabilitation settings preferred.
Strong skills in clinical assessment and treatment planning, along with excellent communication, organization, and interpersonal abilities.
Knowledge of Medicare / Medicaid billing and California regulations required.
Typical workload includes both subacute rehabilitation stays and long-term care residents.
Therapy intensity may reach up to approximately 3 hours per day depending on tolerance.
Facilities often expect high productivity standards, usually around 80-90% of available work time billed to therapy, making proficiency in documentation and patient scheduling essential.
Work hours may fluctuate : quiet periods followed by days with multiple new admissions and intensive therapy sessions.
Flexibility and adaptability are key.
Building strong relationships with nursing and CNA staff is vital; collaborate respectfully, avoid interrupting med passes, and support staff when possible.
