Job Summary : As a Registered Nurse (RN) at our Skilled Nursing Facility, you will play a crucial role in delivering patient-centered care to our residents. You will be responsible for assessing, planning, implementing, and evaluating nursing care for residents, ensuring their physical, emotional, and social well-being.
Key Responsibilities :
- Patient Assessment : Conduct comprehensive assessments of residents' health status, including physical and psychological needs, and develop individualized care plans.
- Medication Management : Administer medications and treatments as prescribed by physicians, and monitor residents' responses to ensure their well-being.
- Wound Care : Provide wound care and manage skin integrity, documenting any changes or concerns.
- Monitoring : Continuously monitor residents' vital signs, report any abnormalities, and take appropriate action as needed.
- Patient Education : Educate residents and their families about health conditions, treatment plans, and self-care techniques.
- Collaboration : Work closely with the interdisciplinary healthcare team, including physicians, therapists, and social workers, to provide comprehensive care and support to residents.
- Documentation : Maintain accurate and up-to-date medical records, documenting all care provided and any changes in residents' conditions.
- Emergency Response : Respond to medical emergencies, follow established protocols, and provide life-saving interventions when necessary.
- Infection Control : Implement infection control measures to prevent the spread of diseases within the facility.
- Quality Improvement : Participate in quality improvement initiatives to enhance the overall care provided within the facility.
Qualifications :
Current and valid Registered Nurse (RN) license in state of practice.Minimum of 5 years of nursing experience, preferably in a skilled nursing facility or long-term care setting.CPR and Basic Life Support (BLS) certification.Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal abilities.Ability to work effectively within a team and independently.Knowledge of state and federal regulations governing nursing care in long-term care facilities.Benefits :
