Salary : Competitive compensation, Salary : $75,000 - $85,000, Processed bi-weekly.

Shift Type : Full-time, Monday - Friday, 8 AM to 4 PM (flexible), on-call rotations.

Summary :

We are looking for compassionate Registered Nurses (RNs) to work one-on-one with our patients in their home environment to establish and oversee all aspects of case-managing patient's care. Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.
