Nurse Practitioner : NC Licensed

A company is looking for a Nurse Practitioner to provide remote care for patients with diabetes.

Key Responsibilities

Provide video-based care to patients in the RPM program

Deliver swift, empathic care and assess members' competencies

Determine members' eligibility for the clinical program and handle clinical escalations

Qualifications

Education : Bachelor of Science in Nursing (BSN) & Master of Science in Nursing (MSN)

License : Active state license; multi-state licenses preferred (GA, NC, NY, FL)

Minimum 3+ years of experience managing diabetes patients

Spanish-speaking preferred, with certification required
