Job Description
The Palliative Care Advanced Practice Provider (APP) is a vital member of the interdisciplinary team delivering goal-aligned care in long-term and skilled nursing facilities.
- Conduct comprehensive assessments and manage symptoms such as pain, dyspnea, and distressing conditions in collaboration with physicians.
- Facilitate goals of care discussions, advance directives, and end-of-life planning with residents and families.
- Support hospice referrals, transitions, and coordinate with the care team to ensure continuity and quality of care.
- Provide education to residents, families, and staff on disease progression and comfort strategies.
- Document visits, contribute to quality improvement, and adhere to regulatory guidelines.
- Maintain professional development, participate in team meetings, and support facility-based palliative care initiatives.
