Care Manager

The Care Manager plays a critical role in ensuring the smooth and efficient delivery of care services to clients at Home Helpers Home Care. This role involves hiring reliable and qualified caregivers, creating and managing caregiver schedules, matching caregivers to clients based on individual needs and preferences, and filling in for shifts when needed. The Care Manager's efforts directly support our mission of providing compassionate, family-focused care that ensures client safety, independence, and comfort at home.
The ideal candidate has at least 5 years of experience in home care, home health, or or hospice. CNA / HHA / LVN or similar is preferred. Candidate must have experience using WellSky or other scheduling software.
Responsibilities : Recruiter
- Source candidates through job boards, social media, community partnerships, referral programs, and local vocational schools.
- Screen resumes, conduct phone and video interviews.
- Schedule and facilitate onboarding process
Schedule Creation and Management
Check for open shifts and identify appropriate caregivers to fill them in WellSky.Fill schedules one week in advance while following all scheduling policies.Communicate with the team about open shifts and hiring needs.Handle caregiver requests for time off.Caregiver-Client Matching
Use caregiver training records and care match criteria to ensure caregivers have the necessary skills for client assignments.Schedule additional training or support for caregivers when needed.Schedule introductions for new client-caregiver pairs.Communication
Serve as the primary point of contact for both clients and caregivers via phone and text.Communicate with clients and caregivers regarding schedule changes.Ensure schedule changes work for all parties involved.Ensure all calls and messages are answered promptly.Mark completed calls and texts as "done" after careful verificationNotify caregivers and clients of any schedule changes.Attendance Verification
Check for any missed caregiver clock-ins or clock-outs.Verify clock-in / clock-out completion with clients.On-Call Support (as assigned)
Monitor scheduling and emergency needs for all offices.Answer calls and texts promptly during on-call hours.Resolve emergency situations and last-minute scheduling needs.Communicate clearly with office staff about any necessary follow-up.
