Job Summary and Qualifications

The Case Manager will collaborate with patients and families to identify needs related to discharge planning, psychosocial aspects, and financial concerns, developing a plan that will : 1) optimize self-care capabilities; 2) ensure proper access to services; 3) enhance communication among healthcare team members; and 4) promote quality, cost-effective outcomes. The Case Manager will have the knowledge and skills necessary to tailor plans addressing physical, psychosocial, educational, and age-related criteria.
