Key Responsibilities :

• Conduct patient assessments and complete OASIS documentation accurately
• Plan and deliver individualized care, including medication administration and wound care
• Educate patients and families on care plans and health management
• Coordinate with a multidisciplinary team to ensure optimal care outcomes
• Provide compassionate support to patients managing chronic conditions
