Job descriptionDeliver direct patient care according to the state Nurse Practice Act. Execute care plans developed by registered nurses. Complete timely and precise documentation aligned with the care plan. Educate patients and families on self-care practices related to their diagnosis. Collaborate with other healthcare professionals to coordinate comprehensive home health services. Utilize and prepare medical equipment and supplies with adherence to aseptic techniques. Engage in continual personal and professional development. Offer health care guidance based on patient assessments and care plans. Graduate of an accredited practical nurse or vocational nursing program. Minimum of three (3) years nursing experience, with preference given to community / home health or medical / surgical experience. Valid LPN or LVN licensure in the state. Current CPR certification. Reliable transportation and valid driver's license with appropriate insurance.

We are expanding our home health services and need compassionate professionals to provide quality care to patients in the Chicago area, specifically in the 60654 zip code and surrounding communities.
Location : Chicago, IL 60654 and surrounding areas.
Hourly Pay : $30.00 - $40.00, based on years of experience.
