A community-based outpatient mental health program is seeking a Substance Abuse Specialist to join its Assertive Community Treatment team. This is a clinical role providing recovery-focused, harm-reduction services to individuals with co-occurring mental health and substance use conditions.

- Hours : 35 hours / week
- Program : Assertive Community Treatment (ACT)
Responsibilities :
Manage a caseload assigned by the Team LeaderConduct community-based individual and group counseling with a focus on harm reduction and recoveryComplete all assigned field visits within the standard 7-hour service dayTransport clients when needed (e.g., from hospitals or court appointments)Facilitate substance use-focused group sessions tailored to client treatment plansComplete initial and ongoing substance abuse assessments; adjust interventions accordinglyClinical Documentation & Assessments for Treatment plans, risk assessments, wellness / safety plansSubmit progress notes on the day of serviceComplete discharge planning and conduct 30, 60, and 90-day follow-ups post-dischargeAssist with administrative and team supportCoordinate with medical staff to ensure timely documentation post-hospital dischargeMaintain a real-time Outlook calendar with daily visits and mileageReport any client incidents or critical events in line with agency protocolPerform other duties, as neededQualifications :
1+ year of experience in a Clinical and / or Rehabilitation role / settingMaster's Degree in Social Work, Mental Health Counseling, Psychology, or Rehabilitation CounselingAt least 1 year of clinical or rehabilitation experienceSolid assessment and documentation skillsExperience with Mental Health illness affected populations and / or Substance Use Disorder individuals
