Assesses the nursing care required for the children of the PDHC Program and implement a plan of care as agreed upon by the Care Planning Team. Assists in the supervision of care given by Certified Nursing Assistants and carries out and / or supervises certain direct or indirect nursing functions. Supports Hospital's mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.\n\nJOB DUTIES / RESPONSIBILITIES\n\n1. Participates in the Intake and Admission process of the clients.\n\n2. Performs a complete physical assessment of each client on admission and ensures that physicals are complete as per policy and regulation.\n\n3. Provides direct client care and administers medications in accordance with the physician's orders; updates orders per policy and regulation.\n\n4. Identifies and immediately reports unsafe practices and / or conditions related to resident care including but not limited to : resident abuse, potential to cause physical or emotional harm to a resident, including signs and symptoms of suicidal ideation. Immediately removes any material that may be an actual or potential harm to the resident and gives it to the supervisor.\n\n5. Develops, implements, monitors and revises the client's plan of care based on upon nursing assessments and physician's orders.\n\n6. Monitors client's immunization needs, facilitates compliance and updates records as needed.\n\n7. Acts as a liaison with community physicians and clinics, and other medical / nursing personnel, regarding the child's care and services.\n\n8. Obtains physician's orders in accordance with departmental policies and procedures.\n\n9. Supervises Certified Nurse's Aides and in developing the client's ADL skills and other responsibilities when applicable.\n\n10. Supervises and instructs nursing personnel in care, charting, changes in policies and procedures, and biomedical equipment use.\n\n11. Supervises the maintenance of the nursing office including biomedical equipment and supplies.\n\n12. Regularly reevaluates the client's needs and revises the treatment plan accordingly.\n\n13. Monitors and records participant's response to treatment.\n\n14. Participates in the preadmission assessment to the program and advises staff on appropriateness of referral in terms of medical diagnosis.\n\n15. Develops, maintains, and revises nursing policies and procedures.\n\n16. Communicates all pertinent client care, safety, infection and quality control, medication regimen, and other medical information with the Director.\n\n17. Completes all client care and medication regimen documentation in a concise, accurate, and timely manner in the electronic medical record.\n\n18. Functions as medical resources in case of emergency. Maintains leadership role for staff in terms of medical issues.\n\n19.
